A PageSpeed score is a lab measurement of one URL, loaded once, on a simulated device. It is good at spotting oversized images and render-blocking scripts. It says little about the pages your users actually hit, because most of those come from a page cache, and the cache hides the cause.
How long the server takes to start sending the page. If it is high on an uncached request, the problem is in PHP or the database, and no amount of front-end work will change it. This is the first number we record, and the one we report against at the end.

Some sites cannot be made fast without changing something the owner does not want to change. A site built on a page builder with two hundred templates will not get a quick admin by tuning alone. A site on cheap shared hosting will not get a low time to first byte, whatever we do in the code. A membership site with every page personalized cannot use a page cache and needs an object cache and query work instead, which costs more than a plugin install.
When that is the situation we say it in the diagnosis, with the options and what each costs, before you commit to anything beyond the baseline. It is better to hear that the answer is a hosting change than to pay us to work around it. If the site has deeper problems than speed, such as edits to core files or a PHP version the host is about to retire, we point you to site rescue first, because fixing speed on top of an unstable site wastes your money.
Speed does not stay fixed either. Plugins update weekly, editors upload images, someone installs a new tool.
